Have you read Hitchens' "God Is Not Great: How Religion Poisons Everything"? He addressed religious opposition to vaccines. I believe the example given was Muslim opposition to the Sabine polio vaccine, but it may have included some discussion of ultra-Orthodox Judaism.
I'm familiar with both cases. In both cases it was more of an issue of conspiracy theories about outsiders than religion per se, IIRC.

Good question. Notice who's at the top of the list, though: An antivaccine pediatrician.
For additional authority, Andrew Wakefield used to be a researcher a long time ago (and, briefly, a clinician even longer before that).
I dislike him as much as you both do, but he was a legitimate practicing surgeon and Fellow of the Royal Society, as well as, as Dorit points out, a researcher of no small accomplishment prior to turning charlatan and fraud.
What did he accomplish of note before becoming an antivaccine crank?
