The real mavericks are the ones challenging the inflation of error for a supposed “benefit”. How does mismeasurement benefit patients? What if the potential target, preventable error, is much smaller and is lost in all this noise?

And here you demonstrate the exact problem that I’m criticizing. Extrapolation from estimates and proxies rather than direct measurement. This is bad methodology. This is how you end up with estimates of death from medical error that are innumerate. I’m not impressed.

Yep. Some of these estimates are clearly ridiculous, as can be demonstrated by comparing them to the average yearly number of in-hospital deaths and total yearly deaths from all causes.
