I could never prove it and could be wrong, but I don't think this tactic is quite as new as you think it is.

We’re not suggesting one is worse than the other. (Though the weaponization of social media channels certainly adds measures of intimacy and immediacy.) But our point, evidenced by tracking these attacks the past 18 months: tactics are evolving, and becoming more widespread.

A letter-writing campaign is nothing like what’s happening now. We’re talking real-time, minute-by-minute, war-room-style attacks, moving from Facebook page to Yelp reviews to Google reviews to DOS attacks, in response to whatever defense the provider or practice mounts.

It wasn't just letter writing. It was phone calls, posting fake reviews on doctor review sites, intentionally poisoning my Google reputation, etc. Sure, it wasn't nearly as sophisticated as it is now, but the outlines were definitely there.
