The number he is looking for, however, is ~5000 deaths per year (and decreasing). When you measure, rather than compound estimation errors, you find much less dramatic results.

I once crunched the numbers, and if accurate, then something like 75% of the people who die in a hospital due so as a result of medical errors, according to Gary Null.
