This is utter nonsense. Whoever did the most work (conceptually, carrying out the experiments, and writing) should be first author, with the senior author being the faculty in whose lab the work was done. First authors can most definitely be medical students or graduate students.https://twitter.com/MDinPink/status/1054437340523966465 …

So I’m being told it is inappropriate for a med student to be 1st author on a paper and therefore the fellow/PI needs to be 1st author. Is this true? B/c this fellow has said I could be 1st author since Jan and I’ve put over 200 hour of time in and have done ALL the writing. pic.twitter.com/oWzmVk2qZU

It can be a detriment to a PI to be first author over their trainees - better to be second/third last with other PIs, than usurp the front from a trainee. If sharing order for contribution, use an *

Totally agree with @gorskon. The best medical journals like @JAMA_current @TheLancet @NEJM have authorship guidelines that are based on who did what. I often SUGGEST that my students take the lead on papers. IMHO, ditch the mentor for a new one! #MedEd

Authorship standards are defined by role, not status. See http://www.icmje.org  The corresponding author is often the senior author when a student or trainee is the first author.

Or contributed the most original thought too, right? Lab techs do a lot of work for papers too but sometimes contribute no original thinking....like at all.....ijs

I have seen med students contribute no original thought with high expectations in authorship.

Perhaps in the US, but in the UK and Australia a student is never a first author, ditto a junior doctor - it will always be an academic supervisor or consultant who takes first billing.

What about program directors who want their names as senior authors in everything written by any residents or fellows regardless of whether PD contributed any original thought or work? I say this as someone who is used to the rules of basic science not clinical research...
