FWIW (and this is where I think philosophy can help with medicine), I believe evolutionary teleology, like most forms of teleology, often gets in the way of good science. Though useful as a hypothesis generating heuristic, it easily becomes a just-so story for pet theories. 1/2

Sure, understood. I recommend this @ScienceBasedMed piece by @gorskon for a pretty compelling skeptical take on Seyfried's approach. https://sciencebasedmedicine.org/ketogenic-diets-for-cancer-hype-versus-science/ …

Nope! Read it on the assumption that @gorskon knows what he's talking about.

I'm not an authority on cancer. But...in all modesty...I'm an authority on signs of trustworthiness in people who are making claims in any given discipline. 😌

When you have an army of cross-promoting affiliate marketing keto peddlers promoting eg, this dodgy miraculous stage IV breast cancer "cure" study from Seyfried, yeah... trustworthiness is suspect at best.pic.twitter.com/9H8lRfTSbf

Holy shit. I looked up that article. It proves nothing. Triple negative breast cancer averages a 25%-30% complete pathological response rate to neoadjuvant chemo therapy. This patient is nothing unusual, and this case report isn't evidence for ketogenic diets. @

Actually path CR rates are more in the range of 40-55% for neoadjuvant chemotherapy with triple negative breast cancer, based on several large contemporary randomized trials.

That sounds more like the pCR rate for hormone receptor-negative/HER2-positive breast cancer treated neoadjuvantly with chemotherapy plus trastuzumab and pertuzumab, which can be as high as 50-60%.

Actually Her2 pos pts have pCR rates ranging from 50-80% in modern Her2 directed regimens. Lower end for ER+ and upper end for ER negative. Triple negative pts in 2 large recent trials with 40% with standard 3rd gen regimen, increased to about 55% with addition of carboplatin.
