So I'm not 100% sure what the scope of "UML" is here. Does every class diagram qualify, or is it something about the inscrutable notation for the many details UML covers?

More the latter. But also the tendency to want to stuff more and more detail in the diagrams, and tends towards massive duplication.

Oh, for sure. When I sketch out a design I use a very small subset of the language with a focus on relations over things you might as well read in the code.

Yea. When it turns more into a communications requirement rather than a narrow aid, it tends to pure cost.
