This definitely sounds familiar and I have found or developed different ways to maintain a sense of direction. Making sure I write things down is crucial, otherwise I'll constantly worry about them until they're either done or forgotten. I keep a "to do list" online.

When the work day is over I write notes to myself to make sure I don't forget what I was doing. Starting the day with a standup is useful as well. I take a lot of notes for work and developed a notation to let me know very clearly if it's something actionable.
