How on earth did this happen, and why aren't these people having deputies driving around and finding them, with the proper ballots so they can vote?

Most likely- it happened because people make mistakes. Especially people who volunteer to do this once a year every two to four years. Our entire system of government relies on volunteers with less than an hour of training. Somehow it usually works.
