Hence the rise of self-publishing - one of (possibly the only) good outcome from Amazon. However, that has shown up a need for some sort of 'quality gatekeeper' role - and it leaves the author doing their own initial publicity (bloody hard work!)

Is that accurate? When I sell something to trad, I retain copyright. They're buying from me the exclusive right to print it for some period of time.

In general I retain the right to add stuff to the universe, change characters, and generally use my IP as I see fit. (If there's a non-compete I may be kept from publishing elsewhere for some period.)

Huh? I may be misunderstanding, but I still own my copyright. Penguin and Hachette own the distribution rights to the Rose Throne books they have under contract, but I've also sold Rose Throne short stories to GdM. It's my IP and no one else's.
