Hey Twitter: what are some popular films, books, or TV series where characters' failures were a recurring plot element, rather than their successes? The Walking Dead and Game of Thrones both come to mind, but I'm drawing a blank for others.

I stopped watching around season... 7? Maybe? But it felt like their failures never really changed things significantly. Even if someone died or got condemned to hell, they reappeared the next season or, if a minor character, weren't talked about again.

That’s mostly true; I just meant that, even when the consequences got changed, the plot was still driven a lot by bad decisions
