Yet there are still journals that limit the overall word length of articles, thus limiting methods sections, and that say that *all* material necessary for evaluation of the article must be in the article itself, and not in supplementary or linked.

Journals that continue to have such policies are *promoting* non-reproducible practices.

I'm sure more details were given, but isn't "full protocol based on..." poorly-defined? If you consider the actual trial as the target, won't every protocol exist on a continuum of approximations? So success is individual or consensus based?

Yup, loooots more definition details given in the actual talk, and protocol replication was put into a many-category (nearly continuous) set of bins. The general point was that there were details that were considered important missing from all studies.
