This is a good time to remind folks: in any sufficiently large organization, big mistakes are inevitable. But in any sufficiently serious organization, there are safeguards to catch big mistakes before they make a big impact.

I don't know the issue here. But in general you don't need to be going after interns.

The VP’s and above are really the ones you worry about. Occasionally a rare few break things intentionally for their own benefit.
