    Unwitnessed and/or non-shockable in elderly should prompt us to recognise that their outcome is abysmal (as pointed out in the article). The fact that we rationalise doing CPR (often >20mins duration) is the problem

    When community expectations & industry are aligned, when we have forgotten to die in community with dignity-there is no stopping this , paramedics & health care workers will continue to wear the moral burden of inappropriate care so that rest of world is shielded

    Absolutely Dave. As you well know it is frustrating and distressing for having to deal with these issues in the field, when the conversation should have been had a long time before we end up there. ACDs should be mandatory on admission to nursing facilities.

    In Ireland we have always done “death” well - let’s not lose that. End of life care is one of the biggest quality issues in healthcare at the moment.

    The medical community needs to start having a hard conversation with the general community: CPR should not be the default for patients aged over 80 and we shouldn’t have to fear legal repercussions for not offering it.
