Quark in DS9 ran holosuites that were pretty much explicitly for sex. Some objects in a holodeck aren't holograms but are wholly or partially replicated and are recycled when the simulation ends. A malfunctioning holodeck can recycle its occupants, this was a plot point in TNG.

Considering the holodeck also used the replicator systems to create certain solid objects that could be interacted with, presumably such a system could also be told to work in reverse. Pretty sure the Enterprise's toilets worked similarly. Best not to think about it for too long.
