One of the great mysteries to me, are all the bizarre & seemingly random errors that happen upon the typesetting of a manuscript. Entire pages mysteriously bolded, scrambled text, disappearing references. Why does this happen? What's going on?

How bizarre. And there was no physical scanning of documents in any part of the workflow? Just digital writing to digital typesetting?

all digital, from beginning to end. there is no earthly reason.

It depends on the exact typesetting markup that was used but if you have a mismatched number of opening and closing brackets the software tries to match every { to a } and semi-random italics errors over an entire chapter would be a common result
