One of the great mysteries to me, are all the bizarre & seemingly random errors that happen upon the typesetting of a manuscript. Entire pages mysteriously bolded, scrambled text, disappearing references. Why does this happen? What's going on?
And then of course you contact whoever at the publisher and you get these thoroughly unsurprised and unconcerned responses. "Please see attached for the corrected version". No explanations, no apologies for introducing weird problems everywhere.

I once had a review partially scrambled by a top literary magazine, despite the fact they did no editing and just had to cut and paste my text into their layout. The editor couldn't explain it, and could only shrug.
in the proofs I'm looking at, a bunch of random words have been italicised. in odd semi-random clusters. fifty pages of this absurdity, for stretches of 2-3 pages there are many instances, then nothing for like 10 pages, then it starts up again.
This happens to everything I have ever written, even after it has been edited by professionals multiple times. Grammatical entropy rays is my explanation.
The competence with which ordinary tasks are executed has fallen into the abyss in the last 25 years or so. I routinely see grammatical errors in the NYT, thinks that would be unthinkable in any past era.
Usually something below the shiny GUI craps itself randomly and proceeds to screw the formatting. Seen similar things w Mathematica myself.
