Since 19th c. at least, you have a subset of women generally with typical demographic characteristics (married, middle-aged, city dwelling, etc) who want to assume the role of invalids /sick people, receive the attention of doctors, be diagnosed with an illness.

These women, who are physically healthy, are the driving force behind these diagnoses. Of course, once you have the diagnosis set up, it collects other people whose problems aren't well understood or easily diagnosed/treated.
