Isn't it obvious though? "Vaccine" is a big shock to the organism, some people require medical help after taking them, they are then tested and obviously some end up with a plus.
I think some of it is surely this, but there is also perhaps an increased susceptibility to infection (or maybe more plausibly: the vaxx turn people with persistent asymptomatic ‚infections‘ into contagious symptomatic cases) for about a week after 1st dose
