Two thirds of Covid19 cases which led to severe disease or mortality were in people who had visited a hospital: the main driver of transmission is hospital buildings. That a plurality or majority of severe illness & death from is linked to hospital / institutional care infections: This is clearly true across West. It is also the case with related coronaviruses like MERS (almost all infections in hospitals) & SARS (hospital infections frequent)

In all three cases, early infections were treated in hospitals, with key early outbreaks surrounding these treatments. It is enormously likely that these viruses were selected above all for their facility to spread in institutional medical environments.
