    Dr. Singh, I may be excessive with this, but I think it is CRUCIAL moving forward (I will make sure to include on a docket comment as well.) There needs to be a plan flt continuity of care in place any time a doctor loses ability to RX as patients are immediately destabilized

    My daughter was forced to undergo three sessions of Rational Cognitive Therapy before insurance would authorize surgical correction of severe spinal stenosis. This is STEP therapy at its worst. RCT therapist thought so too.

    As & I wrote, the whole MME concept is junk. I don't agree with a 90 mg max dose, as pharmacogenomic differences must be considered in order to treat the pt as an individual. 90 mg is completely arbitrary. That said, we should aim for lowest effective dose possible.
