I know two cases off top of my head of FBI agents prosecuted who tripwires tripped up, post-9/11.

but deep in the weeds details of what's in place today I don't know. But Snowden leaked FISAs so more tech assessments happened.

he leaked a FISA order which was unprecedented. So they assessed how to prevent that within USG.

the first leak. But all his leaks led to USG looking at electronic access to FISA info etc beyond Hanssen & 9/11 assessments.

But NSA also had not implemented all security fixes by August 2016.

so to repeat, there have been many reviews over the years and "fixes" aimed at restricting or expanding access to info.
