One thing you might want to be mindful of is if other people are leaving as well. Might not be the case, but "oh hey, guess what - me, too" is a different conversation to have and showing that you're aware could be helpful.

Former manager - decide before hand if they can do anything to talk you out of it for the sake of mental preparation. Be brief and direct, take into account what you are working on and how that might impact the team and if you plan to offer 2 weeks (3 can be nice) say so.

it sounds like just assuming you got your last significant raise, increase in your retirement portfolio and extended health benefits for six mths is your best bet here. Leave a note to that effect. Say God Speed to whoever and then push forward to your new and happy life

IMHO it depends on your relationship; if you have a good relationship then face to face is best (even remotely); in more of an arm's length relationship then text-based is more acceptable. Just one opinion.

Agree. Warmth and directness. Keep it short unless you're friends, in which case they'll already know. I once gave 12 months notice. Never less than a month. Softens it a bit.
