Why is dental and eye insurance separate from health insurance

Traditionally, people paid out of pocket for routine dentistry and eyeglasses. Unsurprisingly, these things were relatively affordable. The dentists lobbied hard with big unions to be covered. 3rd-party payer mysteriously coincided with massive increase in cost. No one knows why.

Strangely, to “keep college affordable” and avoid having younger generations saddled with huge debts, the US has repeatedly increased aid to higher education. Mysteriously, costs in higher ed more than swallowed previous aid. Why this happens is also unknown.
