insurance companies and government (medicare, VA, whatever) are paying specifically for treatments or perhaps outcomes that are easy to measure typically the consumer (patient) is not

so you have a sort of multitask problem where "nice experience" is ruthlessly selected against because it is difficult to measure compared to (eg) patient survival, and so not a good goal metric for (eg) public health officials

Last time I was admitted there a was a masseuse and a therapy dog that went into rooms (not at same time). My great-grandpa also used to give free haircuts to patients in the early 1900's.

that weirdly is consumer driven it used to be extremely luxe because fares were subject to price controls and airlines had to compete on amenities now they can compete on price too and it turns out consumers care about that a lot

It's too bad that hospitals aren't more like airlines where consumers pay their own bills, and thus there has been minimal cost cutting and luxury remains the order of the day.
