In many times and places obesity was or is high-status. No obvious reason that can't ever change, though you may well be right that it's not a change campaigners are able to *cause*.

I definitely think it matters object level, as someone down 20 pounds (out of a 30 goal), for thinking about this decision. I'm losing weight purely out of vanity, but if it actually does decrease my life expectancy, I'd at least like to know, if not possibly act on it.

IIRC, the findings were that BMI of 25-30 was associated with slightly *lower* excess mortality, while the excess deaths for obesity (BMI>=30) were about 1/3 of what was previously reported. I wouldn't take those findings as personalized medical advice, though.
