I've been wondering about TNF alpha immunosuppressants and COVID-19! A close friend is on Humira - in fact she's on a large group video call about Crohn's and coronavirus right now. Any links about the potential effects there?

so this is actually super fascinating to me, and i can go into a bit more detail of what i know (as speculative as it is): it SEEMS like prognosis for COVID-19 is all about your ability to produce and maintain killer T-cells to go around killing cells that are virus-infected.

people who run low on T cells tend to have a very poor prognosis, and adaptive immune system function is highly correlated with success in fighting off COVID-19. this matches the age relationship, because thymus function massively declines with age (see thymic involution).

now, to get into more speculative territory: why are the T-cells running out? if i understand things correctly: there are signalling pathways that trigger T-cell apoptosis. they largely exist to prevent the T-cells from mass-targetting healthy tissue by mistake.

it's possible that the virus is doing something in order to try to trigger or enhance this pathway. it's also possible that it's simply a matter of how much progress the virus makes before the T-cells really get started, and if the virus is too far ahead, they're just doomed.

AFAIK, the TNF-alpha pathway *is the pathway that suppresses T-cells like this*. so arguably, it might actually be BENEFICIAL to suppress TNF alpha, in order to enhance adaptive immune system function.

anecdotally, i have heard stories of people taking sub-psychedelic doses of DOI for its TNF alpha suppression properties, as a poor man's Humira substitute (it's a picomolar, yes, picomolar TNF alpha suppressor http://jpet.aspetjournals.org/content/327/2/316 …)
