it's to prevent people from going uncovered without penalty until they need substantial medical care, then signing up the moment they learn they're going to need to make some major outlays

they could have permitted enrollment at any time without this risk (well . . . sort of) if they had allowed exclusion of preexisting conditions, but of course this would have its own problems designing an insurance system is frequently a very hard problem, economically speaking

we don't really have an insurance system at this point more like a subscription service
