The disappointing thing here is that we'll happily blame a power cut rather than say "the people who developed the software didn't take account of this common failure mode". /cc

in my experience, these sorts of things are usually more the fault of IT/system administrators than people who know anything about developing software..

Yes, I expect so (tried to capture that with "developed" rather than "wrote" but maybe still not quite right). Ultimately, the error is due to a human, though, and software will be better when we accept that.
