Oh, it looks like the person was using a picture to represent the fact that Elliott Page has had a masculine presentation for most of their life. (The bike is incidental, of course.)
Of course you're right that a masculine presentation isn't necessarily indicative of gender identity, but I'd say its fair to consider a GNC presentation a sign as long as you don't think of it as determinative.
The problem is when that sign is used to medicate children who are too young to understand the impact an experimental treatment like the one used for transitioning will have on their future life. (Also presentation doesn’t change your sex, no matter how masculine, a girl looks.)
