That's the issue. Identifying as a different gender isn't a disorder, but the dysphoria that often accompanies it is.
Yes, we seem to be getting back to it! So, now, what if a trans person refused to follow sexist dress codes because they had a strong sense of justice and wished to live freely and advocate for a better world, but felt distressed by the discrimination and financial insecurity?

Presumably the psychiatrist would have a long discussion with them and probe their motivations for their actions. If they surmised that the patient was doing it as an act of political/social protest rather than from a personal discomfort with their gendered self-conception...

...then they would have reason not to diagnose them with a disorder. This is assuming a competent doctor who takes this seriously. What you'd encounter in real life varies tremendously. The spirit of the guidelines is to provide a general framework.
