Isn’t the analogy: Trans:GID::persistent hallucinations:schizophrenia. And in this case there is a significant group saying that it’s offensive to say that you can’t be trans without it being concomitant with extreme distress (see the “transmedicalist” issue)

Take ADHD or bipolar disorder, for example. If there is no impairment and no distress how do you establish these as "disorders"?
As I was saying above- impairment is distinct from distress. The case of ADHD is a great analogy, in fact. You can experience no distress, but be experiencing impairment due to lower test scores.
