Is this happening across all browsers? Some troubleshooting steps may help–signing in/out, clearing cache/cookies. Also, a few things to remember–you must have a verified account, a video cannot appear in more than one series playlist, only videos uploaded by you can be added.

This was happening across separate computers entirely, and we do have a verified account. It was to be in a single series playlist and was one we uploaded. Ultimately we were able to get it to work by waiting for the video to be published and adding it from the watch page.
