Well, I ended up restoring it to 9.3.5 (RIP iOS 6). Turned out to be a 32GB AT&T model, and is in fully functional condition.
Just got this iPhone 4S off eBay for $14... Any chance of removing this passcode? Or will I be forced to restore to 9.3.5?pic.twitter.com/NXgy04fIhR

It looks like the baseband may also be bad. Doesn't detect SIM insertion, and sits at "Searching..." in the status bar.
