Why does every project, every piece of code feel so incredibly bodged? How can software engineers get on a plane and trust their lives to millions lines of code once you’ve seen what code usually looks like?!

Well, I'm doing a long nav flight early January. Guarantee that the certificated nav avionics will crash. Fortunately, I do not depend on it. That is, after four times phoning the (only) aeronautical chart provider in AU about missing or buggy charts in the latest release.

I assume you mean across all classes of aircraft? Or just small craft like the ones you fly?

I can only speak to the incidents I know of, in various sizes of aircraft. All aircraft are regulated by the same regulatory body, and incidents reported by the same safety bureau. There is no distinction, except that which I experience or reliably learn of.
