Failure to warn is simply a different flavor of a product liability claim. The other two are design defect and manufacturing defect. Failure to warn is the most appropriate for this type of exposure case.

If someone drink 6 liter of water he will most likely die. Then you can sue whoever didn't put "do not drink more than 3 liter at a time" on his bottle and win the case for "fail to warn consumer about the risks". This does not mean that water is dangerous.
