I wonder if this is cultural inertia from the days where all periodicals had similar sounding names ("The Accounts of Philosophical Reviews, with approval of the king, &c."): it must have been hell to identify them without a location name.

Does any style require it (or the publisher name) for periodicals though? In the styles I'm familiar with, it's only for books. I assume that—pre-internet—lesser known publishers (and thus their books) were easier to track down if you knew where they were based.
