If a disease kills too high a percentage of those it infects it can't spread. Consider Ebola, it kills a high percentage of those it infects, and does so rapidly. It's a horrific disease but dies out once it has run though a village or those infected are isolated.

An effective infectious disease (from the viewpoint of the pathogen) is one that doesn't kill too many hosts, has no symptoms for some time after infection (bonus for being infectious during this time), and passes readily from host to host. How the Spanish flu spread worldwide.
