A2. Slowly and painfully :D Initially, it was basic manipulation - over time, the concepts of "quality" and "user" have gained traction, as has the simple truth that the metrics that matter focus on Money, not Ranking or Traffic.

It's shifted from a "dark art" that was little understood,
to a fairly commonly encountered term and concept.
But with each passing year, the breadth of things covered seems to expand.
From Keyword Research and Inclusion, to Crawlability, on to Load Speed etc.

Fortunately, I think the sector has recently hit a tipping point. It seems more people that do SEO realise what they should be measuring, focusing on and that the name of the game isn't chasing algorithms or obscure term rankings...

... but instead getting the right traffic, that will benefit the business (be it via financial transactions, in-store visits, brand awareness, reputation etc.). I think many of the major changes are tied to Google Algorithm changes/releases, such as Mayday, Panda etc. =
