Historically MSFT did ~40/40/20% split of dev/test/PM. Factor for managers, that's ~300 coders at best.

The inefficiencies come from trying to coordinate that many ppl. Features don't work with each other etc

Look at add-ons/plugins for example. IE _still_ hasn't gotten that right. Then compare to Chrome.
