Has something ever failed in the field due to a bug in a general-purpose CAD tool? (I.e. not the Hartford Coliseum.)

@paulg A bug in an electonic CAD tool made a chip dead on arrival, by confusing greater than with lesser than. Despite all the testing.
@paulg I can think of a few cases like @alain94040's. DoA isn't so bad. The worst are subtle issues that aren't caught until after release.
