I keep reading about incidents like this and every single time I'm left wondering "how did this go on for so long?" there are no outside observers in those classrooms? the students are all so troubled that the ones who see it for what it is are in the minority?

The professor made the difference here -- instead of abusing a few students at a time, like a respectable normal academic, she got greedy and needed dozens under her thumb.
