one of the main causes of population explosions post industrial development is the lagtime in cultural update to adjust to low infant mortality rates. the medical uplift happens almost instantly but it takes like 40-60 years for the marriage and mating habits to catch up.
actually reductions in fertility frequently happened before any reduction in infant mortality
(and were presumably causal of that reduction)
