Americans are gigantic and yet they're not being punished for it: blood pressure and heart attack mortality have both declined even as obesity has increased. This has happened in the US, Europe, Japan, and Korea despite very different overall diets.

Changes in obvious risk factors like smoking fail to explain a majority of the decline: I suspect changes in infection and inflammation to be important, as evidenced in the simultaneous decline in average body temperature.
