FWIW, I initiated and advocated heavily for every promotion I got. Didn’t get them several times. 
At Square, we reassessed everyone’s level on a regular basis, and proactively promoted people.
Managers have to set aside time for explicitly considering promotions and make sure it doesn’t slip through the cracks.
