There are undesirable ways that multiple hierarchies can interact for mutual benefit that holds back disruption/change that would normally happen. Think of monopolies, duopolies, price-fixing, etc. Optimally, we need a balance of centralized and decentralized decisions.

and the failures of these institutionalised hierarchies are due to failures in the measurement of consent & competence. also the inflexibility of change as a response to decision-making. dynamic systems need to have this functionality built-in. rigidity is often weakness
