It’s presented as an alternative to “male” or “female” (or “other”), which could be seen to imply that transgender people who identify as male or female don’t truly belong to that gender.

(I mean, it’s paper, so nothing’s stopping you from checking multiple boxes, but it appears to be intended that you pick one.)

I feel like a doctor’s office is one of the few places where this is actually very relevant information. Both birth gender and transgender procedures have huge effects on how one’s body is affected by different ailments and treatments.

With that said, the questionnaire could definitely be designed better. Maybe asking explicitly for birth gender and then asking if the person is transgender as a separate question?

Yes, my problem is not with asking about it but with presenting “transgender” as a choice for “gender”, seemingly mutually exclusive with “male” or “female” (or “other”).

I agree that it is sub-optimal UX, but I think that it is less an attempt to exclude transgender and more a carryover from a diagnostic viewpoint. Doctors will diagnose transgender differently than cisgender based on their phisiology. In that way they would be distinct.

I am not a doctor, though, so perhaps there isn’t as much difference diagnostically as I imagine. Take it with a grain of salt.
