First rule of design is that you never know the design until you actually work it through carefully with lots of real-world experiments. So the answer to "how would I do it" is I would go and I would do it :)

And that is true the other way around as well - the reason Unicode is not good is likely because they didn't spend nearly enough time writing code to use it as they were designing it - if they had, they would have seen how poorly conceived the design is in practice.

Yes - at a bare minimum, they should have taken many example texts from around the world, and tried to achieve the minimal program that could display and edit all of them. That would have told them how many significant problems their spec has.
