    Very much needed. 1. IT supports essential but in itself will not solve the problems. 2. Key issue for medication is including reason for meds stopped or changed so colleagues know if intentional or not. 3. Medicines reconciliation addresses this but rarely in place at d/ch

    Busy a.m. -four new diagnoses- Type Two Diabetes , Psoriasis, Mitral valve disease and thyrotoxicosis - two post hospital patients- neither had a Discharge letter Very difficult to know if meds are changed or forgotten!! Communication between Primary/Secondary Care has to change
