2/ Or even more mind-bendingly, there isn't just a "root" cause but really a combination of things that wind up being bad together but not in isolation. Thus having a habit of constantly reevaluating your conclusions about what works and what doesn't is crucial.

3/ It's also a big pain in the butt. But that's the nature of dealing with things this complex. You have to appreciate that you're going to be wrong a lot. Probably most of the time in some way.
